Early drop-off is encouraged for those bringing in forms without an appointment.
Fees must be paid at pick up.
Forms will be processed and returned one week from the date of drop-off. This includes any forms that require written information or a doctor’s signature.
It is advised you call the office before picking up forms to be sure your documents are ready.
Please fill out any information that can be completed prior to visit or drop-off. This may include:
Name of Patient
Date of Birth
- Any questions pertaining to the patient's past medical history.
